The Social Support Program in New Jersey provides social work care management services for Summit Medical Group, one of the largest physician owned multi-specialty groups in the country. I have more than 30 years of experience providing care management services to adults with severe and persistent mental illness and older adults with complex medical conditions. My work settings have included healthcare, not for profit and government agencies. I have a background in ministry and management, and enjoy working with people on all levels.
