By implementing and sustaining these high quality team processes enables hospital units to partner with patients and families and achieve consistent interdisciplinary patient-centered care at the bedside. Our team consists of doctors, nurses, engineers and change-makers. We've been working with front line care teams for over a decade on these processes and have learned so much about what it takes to succeed with team-based care.We've developed a methodology to enable the ACU care model to be successfully adopted by many hospitals in multiple countries. Hospitals have reported better patient-centered care measured by significant improvements in patient and staff satisfaction, lower mortality rates and significant reductions in length of stay leading to large cost savings and better care outcomes.I became absolutely passionate about this when my father suffered from a communication error between his care team and was severely harmed in the process. I was working with the team at the Centre for Healthcare Innovation (Alfred Health) around a communication protocol initiative at about the same time that this happened to my father so it was especially obvious to me that we ought to be able to do better.
