Current• Conduct comprehensive physical examinations and health assessments for patients of all ages, ensuring thorough evaluation and accurate diagnosis.• Develop individualized care plans, integrating preventative strategies and evidence-based practices to manage acute and chronic conditions.• Prescribe medications, therapies, and other interventions while monitoring their effectiveness and adjusting as necessary.• Provide patient education on lifestyle modifications, disease prevention, and management of chronic illnesses to promote long-term health and well-being.• Coordinate care with specialists and other healthcare providers to ensure seamless transitions and continuity of care across different settings.• Utilize electronic medical records (EMR) for accurate and timely documentation of patient encounters, treatment plans, and follow-up care.• Perform routine screenings and diagnostic tests, interpreting results to guide clinical decision-making and patient management.• Engage in regular follow-up with patients to monitor progress, address concerns, and adjust treatment plans as needed.• Participate in quality improvement initiatives and practice-based research to enhance care delivery and patient outcomes.• Mentor and supervise nursing students and new staff, fostering a collaborative learning environment and promoting professional development within the practice.
