Step Inc. Human Services Feb 2018 - Aug 2019
STEP Inc. works with individuals who have development disabilities assisting individuals and their families in accessing the 0208 Waiver as well as supporting them in different areas of life. I work in the Family Education department. My clients are ages 3-21 and are at risk for being developmentally delayed or are developmentally delayed. My job is to support the families through education, informal counseling, home support, and case management services. I often travel to meet my clients, contact them by phone call, and make phone calls and coordinate services on their behalf. I coach parents in strategies and implementing tools in the home to support and advocate for their child. I develop treatment plans (IFSP's) with the family's and coach the families in the step to achieve the goals developed. I attend team meetings, trainings, and travel to meet about 25% of my clients. I recently developed a program to track our clients and the outcomes they achieve thus measuring the success of STEP Inc. I enjoy being creative and tackling a challenge, I believe that this quality will make me an asset to other organizations as well.
  • Aware Inc. Human Services
    Youth Case Manager
Aware Inc. Human Services May 2017 - Jan 2018
I worked closely with the Successful Starts clinician as well as the community side case managers. I attend weekly meeting with Community side and successful starts as well as individual meetings with the clinician and my clients. I am responsible for developing treatment plans in conjunction with the clinician, AWARE services, and individual needs of the clients and their families. I also coordinate services within AWARE and in the community, advocate for, and make referrals as needed on behalf of my clients. To this end I conduct research, make phone calls, attend meeting, doctor appointments, and school appointments. I talk to counselors, teachers, daycare attendants, and other professionals and supports in their lives and treatment. I schedule and maintain my schedule independently; I see all my clients at least once a month and updated treatment plans every 90 days. I do my best to provide the right services to the right people at the right time. AWARE is a great company with great people, I enjoy working here and am learning a lot, I feel that AWARE Inc. has assisted in my professional development and skills that I need to become an asset for a future organization.
  • Tumbleweed
    Youth Advocate
    Tumbleweed Apr 2016 - Nov 2016
I worked day and overnight shifts, sometimes 10+ days in a row. My duties included checking runaway and homeless youth in and out, providing hot meals, making sure they have access to appropriate clothing, maintain the peace and safety of all people, youth and staff, in the building. I made sure youth had access to a hot shower, hygiene products, and clean bedding. I conducted intake interviews and ID checks. I conducted case management services by connecting youth to appropriate departments or needed community resources and services. For example, I connected sex trafficked victims to the sex trafficking counselor, youths with substance abuse to treatment centers, I made phone calls to Dress for Success on behalf of clients, I assisted in the development of resumes, and searches for available housing covered under section 8, or anything else I could find to fit their needs. I spent time with each of my clients getting to know them, listening to them, and building relationships so that they could trust me. My responsibilities also included Administration duties such as filling out intake paperwork, writing DAP reports, and end of shift documentation. I also diffused arguments, assisted sick youth, I cleaned, cooked, and had organizing responsibilities. I had frequent contact with the police department, psych center, and community counselors. I loved and still love the mission of Tumbleweed and the demographic served. As I worked at Tumbleweed I wanted to do more than meet their immediate needs, I wanted to meet their long-term needs, I wanted to help them develop self awareness, eliminate barriers, and achieve their potential. I left Tumbleweed to gain the professional skills necessary to be able to fulfill my dream of meeting adolescents where they are at and working with them to become successful. I left Tumbleweed to work for AWARE to get one step closer to achieve this goal.
