Developmental Disabilities Services Division (DDS) offers a wide array of community services for individuals with developmental disabilities and their families. Services are individualized to meet each person's needs. DDS services are funded through Medicaid Home and Community Based Services Waivers and through State funds. Case managers assist people with developmental disabilities gain access to needed medical, social, educational and other services.Responsibilities • Assess specific needs of each individual assigned to caseload.• Work high-profile, complex cases from multiple regions across the State. • Work with upper management positions within the division to ensure cases are within policy limitations. • Lead teams of 20+ members in transitioning individuals from institutional settings to community based living. • Coordinate services with residential, vocational, psychological, healthcare and all other professionals on each individual’s team.• Develop Individual Plans that prescribe the services identified to meet those needs.• Monitor Plans to ensure that services are being provided as prescribed and continue to meet the needs of the individual.• Plan, develop and lead team meetings to create addendums and adjust plans as needed in an expeditious manner. • Complete monthly monitoring visits to each individual’s home and vocational site to monitor services being provided.• Complete monthly and quarterly progress reports by compiling data and reports provided by professionals working with each individual. • Regional Trainer for Person Centered Planning.
