-Patient Screening of nutritional/medical status utilizing medical history, physical examination, biochemical and other objective data including age. Responsible for establishing nutritional goals with program patients, and identifies patients who are or may become malnourished or at risk.-Perform initial and follow-up assessments of program participants following state and national guidelines. Determine and make recommendations to program participants and physicians and documents in medical record specific nutrient needs of patient, integrating the principles of nutrition for the stage in the life cycle disorder or disease being treated.-Instruct patients in various ages (and their family members or significant other as needed) on prescribed diet restrictions and rational, individualizing meal plans and counseling sessions documenting progress and education in appropriate part of the medical record.-Check blood sugars of patients with diabetes, and provide appropriate counseling regarding results as needed.-Participate in interdisciplinary team conferences serving as consultant on nutritional care.-Provide nutritional education as requested by staff of the medical center and serves as a program representative in the community.-Organize and perform work effectively and efficiently.-Maintain working knowledge in the use of personal computer workstations and Microsoft Windows environment for delivering patient care to include placing orders for clinical services, reviewing results from clinical tests and documenting delivered care.-Demonstrate standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence.-Aide in program development and expansion.-Assists in daily operations of programs as needed.
