Provide medical case management to injured workers through communications with patient, physicians and other health care providers, referral sources and employers. Make recommendations regarding health care resources. Assess, plan implement and evaluate interventions and treatment plans for appropriateness and medical necessity based on the ODG guidelines. Negotiate and coordinate services and equipment for cost effectiveness. Attend doctor’s visits, and other healthcare provider visits such as hospitals or long term care facilities for team conference and discharge or long term care planning. Utilize nursing knowledge to discuss treatment plans and alternative treatment plans with providers for best possible outcome and cost effectiveness. Work as a dependable member of the team to deliver excellent customer service.

Dallas/Fort Worth Area

Work as a team leader for the purpose of enhancing the quality of care and comfort of terminally ill patients in collaboration with the Patient Care Manager and the Interdisciplinary Team. Assessing, planning, implementing, and evaluating medical and nursing interventions as necessary to ensure patient safety, comfort and quality of care. Act as a role model providing leadership and support to the hospice staff. Adhere to Company policies and procedures and regulatory compliance requirements and directives. Manage related expenditures in a fiscally responsible manner in accordance with the Company’s budgets. Travel as necessary and fulfill on call duties as necessary.

Work as a team leader in collaboration with multidisciplinary team and other healthcare delivery partners as indicated to optimize healthcare delivery and maximize patient outcomes. Provide ongoing management of patients as they move along the continuum of multiple levels of care with chronic and complicated conditions ensuring their safety, holistic well being and providing cost containment by delivering the necessary services in the least invasive setting without compromising outcome.
