I am an efficient, thorough, team player with great time management and rapport building skills. I also adapt well to change and function skillfully within inter-disciplinary teams. I graduated top of my class with a 4.0 grade point average from the Texas A & M-Commerce Social Work Department. I have provided social work services to facilitate solutions to medically related psychosocial/environmental problems of nursing home, rehabilitation, home health and hospice patients and families for 8 years. I have also ensured the continuity of patient care by implementing interventions that include ongoing assessment, counseling, referral, aftercare, placement, information, and guidance during in/out patient treatment for drug, alcohol and dually diagnosed patients.
