CTC will facilitate transition of patients between referring physician or host hospital to LHC agency. The CTL educates the patient and family on HH practices and confirm acceptance of the agency based on the ability to meet patient needs.

Generations provides adult supportive residential facilities throughout the state of Tennessee. Generations provides residential care, treatment and rehabilitation for mentally ill service recipients. Generations provides personal care services, med management, training in community living skills, medical services, social services and mental health services. Generations also provides outpatient services for individual, group, and family therapy.

WCNH provides 24 hour skilled and ICF nursing.
