- Provided consultation, data analysis and organizational support to 12 independent practices in the Pediatric Physician’s Organization at Boston Children’s Hospital (PPOC)- Assisted and advised PPOC practice staff on methods, tools and techniques for monitoring and assessing quality of care using the Clinical Microsystems approach- Supported implementation and evaluation of quality improvement projects between PPOC practices, specialty care and community-based services- Assisted in the development of an annual customer survey to evaluate quality improvement program activities- Co-chaired a Complex Care Management committee in charge of developing a consultation model to ensure that all care management services and Medical Home activities are seamlessly integrated with the patient/family, the PPOC, Boston Children’s Hospital and community-based services to support the needs for medically complex patients throughout the PPOC network- Participated in weekly multidisciplinary team meetings to provide clinical support and supervision to nurse complex care managers, community health workers and administrative staff- Assisted in the development of a real-time support program for PPOC practices struggling with providing care to patients and families with complex medical and behavioral health conditions- Designed and implemented a new onboarding process for clinical and non-clinical care coordination staff hired by a PPOC practice- Served as project manager for network-wide Medical Home Care Coordination quarterly trainings

- Designed and implemented the first integrated behavioral health service at the health center’s main clinic and teaching site- Participated in team-based quality improvement projects to help maintain NCQA level 3 Patient-Centered Medical Home status- Provided brief evidence-based behavioral health interventions utilizing problem-solving, cognitive behavioral and motivational interviewing techniques- Participated in an NIH-funded community-based participatory research project that looked to identify barriers and facilitators of mental health service utilization among Latinos

- Developed and implemented a group medical visit model for the Office-Based Opioid Treatment program’s Suboxone Clinic- Developed clinical and didactic longitudinal trainings focused on evidence-based behavioral health interventions for family medicine residents- Provided skills-based training activities aligned with core competencies developed by the Accreditation Council on Graduate Medical Education- Successfully developed and performed cross-cultural and patient-centered trainings for family medicine residents and residency faculty
