- Interview patients diagnosed with reportable diseases to assess and mitigate possible sources of infection, provide health education, implement control measures to interrupt transmission, and document outcomes through the state reporting system (CalREDIE)- Identify and notify contacts exposed to a wide variety communicable diseases while maintaining confidentiality; coordinate symptom monitoring, laboratory testing, and post-exposure prophylaxis for contacts as appropriate- Investigate outbreaks and exposures, facilitate specimen collection and laboratory testing, and provide infection control recommendations- Multi-drug resistant tuberculosis (MDR TB) grant funded project including daily observed therapy (DOT) and thorough contact follow-up for MDR TB cases- Perform phlebotomy and PPD/tuberculin skin tests; organize and participate in TB screening clinics for large exposure sites; case management for TB immigration clearance - Provide education about communicable diseases and prevention through one-on-one and group discussions, presentations, document/resource creation, and program-specific web pages- Case management for the Perinatal Hepatitis B Prevention Program; chronic Hepatitis B and C reporting- Obtain and input updated contact information into the California Health Alert Network (CAHAN) system to facilitate urgent communication to local medical providers
