Develop and implement CCMR practices. Organize, maintain, and support the Readiness Center on campus, and provide a wide variety of Readiness information, materials, processes, and supports for successful transition of high school students into post-secondary opportunities. Mentor and assist students with preparation activities for job placement. Facilitate various postsecondary visits for high school and middle school students. Track current and future College, Career, and Military Readiness (CCMR) for all junior and senior level students. Provide career and college entrance information and assistance to students, teachers, parents, and counselors. Assist students with scholarship, federal aid and college applications. Maintain records and documentation. Provide quality management oversight for CCMR indicators for our campus.

Develop and implement lesson plans that fulfill the requirements set forth by the district. Prepare lessons that reflect accommodations and diversified learning in the classroom. Plan and implement appropriate instructional and learning strategies, activities and materials that reflect understanding of the student’s learning styles and needs. Conduct assessments of student learning. Gather and implement data associated with student goals and objectives. Work cooperatively with co-workers. Build and maintain appropriate and purposeful relationships with students in order to promote growth and learning.

Provide strength based, solution focused, client driven intervention services to individuals, families and groups. Create and implement action/behavior plans monthly for the purpose of goal setting and monitoring client progress. Conduct skill based training with emphasis on such topics as communication skills, problems solving, critical thinking, anger management, parenting education and conflict resolution. Coordinate with community agencies and professionals as needed, including MHMR, juvenile probation, Uvalde Memorial Hospital, Uvalde County Judge and Texas Department of Health and Human Services, etc. Participate in educational meetings with teachers, administrators and parents to develop and implement behavior plans and/or make necessary referrals to outside agencies.

Investigates reports of abuse, neglect or exploitation of elderly or disabled adults to assess the urgency of the problem by interviewing clients, family members and others to gather social, functional, physical and mental-emotional information for evaluation. Manage caseloads of 20-50 clients. Provides services after normal working hours and on weekends to maintain 24hour coverage, including receiving reports of alleged abuse, neglect or exploitation. Determines temporary or long-term action to be taken to remove or lessen an immediate threat to the life or health of an adult protective services client to include counseling clients and caregivers, providing information and referral services, testifying in court to seek emergency protective services, finding alternate housing when necessary, and providing on-going services. Develops individualized service plans to meet the needs of the adult protective services client and maintains detailed documentation on each case. Makes presentations and participates in community awareness and related activities to inform the public about the program. Prepares routine, statistical, and special reports for management. Create and file affidavits for court. Knowledge of Medicare and Medicaid programs.

Responsible for implementing and coordinating state service for at risk children while accessing parental and family involvement, community resources, and safety factors of the home. Adhering to all state laws and agency policies to implement safety programs. Applying direct one on one communication with parents and family members of a diverse population to illicit required information and give critical information regarding minimal standards of safety for children. Apply elements of critical thinking to improve communication outcomes during interviews and investigations. Make referrals where appropriate to remove or reduce immediate threat to the safety of the child, testify in court, place children in protective custody, refer families for immediate crisis intervention. Compile reports for court. Create and file affidavits for court. Document contacts according to agency policy using various state agency software including Microsoft Word, and IMPACT program. Use HR software to record work time, request benefits including travel and education time. Maintains clerical files, and data entry as required. Use networking applications as needed. Develop and maintain professional relationships between the agency and community organizations including judicial officials, legal resources and advocacy centers. Performed investigative work independently, ability to prioritize workload and report critical information within set timeframes.
