Responsible for assessment, development and coordination of services for each of their individuals and families in accordance with the OPWDD MSC Vendor Manual and/or FSS Contract.Assess the needs of the individual and develops an effective plan of service with the individual and parent.Make ongoing referrals as needed and assists in linking the individual to those services.Monitors existing services to ensure appropriateness and ongoing need.Coordinate the delivery of services with individual/family, other agencies as well as other involved parties (i.e., schools, day programs, etc.).Support individuals receiving services in crisis intervention and necessary follow-up.Maintain a minimum of 3 face to face contacts with individual per year in accordance with the MSC Vendor Manual.
