Acting Team Leader for the Western CTG ITC team based at Port Adelaide aswell as CTG South Team Leader,

The ITC program, through the Closing the Gap scheme, contributes towards improving health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through access to care coordination, multidisciplinary care, and support for self-management and improve access to culturally appropriate mainstream primary care services for Aboriginal and Torres Strait Islander people.The Closing the Gap Team Leader is responsible for working collaboratively with patients, general practitioners and practice staff and Aboriginal health services to provide appropriate multidisciplinary care coordination and services for Aboriginal people with a chronic condition.

Working as a solo practice nurse in a small family run GP service in South Adelaide . My main duties were chronic Disease Management, Health assessments, Recalls , Immunizations, Wound care , Spirometry, ECG and Venepuncture .

I worked supporting general practices in the southern Adelaide region advising on services available to them from the Medicare Local aswell as ,Medicare MBS , Digital health , Clinical Audit Tool, Immunisations .I also worked some of the time as the Immunization Coordinator assisting with enquiries around catch up vaccinations , cold chain ,AIR and general queries.

I worked as practice nurse supporting the General practice team. I assisted with care planning ,health checks and clinical nursing such as wound care , spirometry , vaccination, ECG , observations and triage of patients .
