Provide case management through telephonic care management, care planning, implementation, care coordination, monitoring, and evaluation to ensure member receives services and supports required to meet medical, behavioral health, psychosocial, educational and cultural care needs. Develops, facilitates, and communicates a person-centered plan of care in active participation with the member, his/her significant other, primary caregiver, the primary and attending physicians, and various other providers involved in the member's care. Identifies intervention to assist the member in meeting short and long term plan of care goals and request authorization for services based on agreed upon member care plan.

Assist in the program development of a new inpatient drug and alcohol rehabilitation center. Create lesson plans for educational groups for patients. Educate patients on medications, and the science of addiction. Provide medications as ordered by physician. Handle entire aspects of patient admissions to facility from a medicinal - nursing standpoint . Administer vaccinations, complete lab work, and monitor patient's health and treatment goals.

Provide professional nursing care to emotionally disturbed, mentally ill, substance abuse, and general Med- Surg patients for Behavioral Health and Recovery Services; to work with clinical and case management staff to coordinate care and assist in treatment plans for clients; and to perform related work. Create and implement plan of care, preform thorough general and specialized assessments, administer medications and maintain therapeutic levels, monitor patients for improvement and implement interventions as necessary. Document all patient information in EMR. Provide education to patients at appropriate level. Ability to use professional judgment to ascertain and to apply appropriate intervention techniques with patients in crisis. Responsible for performing clinical tasks such as taking blood samples to help with diagnostic tests and connecting patients to medical equipment. Perform electrocardiograms, which measure the electrical activity of a patient’s heart.Quickly and accurately obtain vital signs such as blood pressure or respiration rate and ready patients for various emergency procedures.Putting on splints and helping with simple sutures , take care of medical equipment and ensure machines are working properly. Make changes to patients’ medical charts as necessary and keep emergency room supplies stocked.Attend to incoming Traumas by air and ground , Stabilize patients , Implement CPR , Transport patients , maintain communication between all staff and patients . Wound Irrigation and dressing applications , obtaining visual acuity . Priming and spiking IV fluids as ordered.

Handle clerical and other administrative tasks for a busy Hematology Oncology unit. Assemble patient records and file them for consistent, easy access. Create labels and do other paperwork and assist in finding files as needed. Order supplies as directed by doctors and nurses and ensure those supplies are properly delivered. Answer telephones and guide patients and visitors. Transcribe Chemotherapy orders , input doctor orders into the computer , facilitate communication between all staff . Ensure patient needs are being met.
