Inpatient Call Back: Call discharged in-patients within 48 hours of returning home to ensure ability to receive new medications, medical equipment, and home care. Ensure that patient is aware of follow up appointment and transportation is arranged, and has no new needs or concerns. Assess and coordinate the implementation of care if needed after returning home.Coordinated Care Network: Chronic Disease Nurse; Performs medication reconciliation. Identifies and implements assistance (family, pharmacy, technology) needed to improve medication compliance. Provides patient and family with disease specific education to help them understand condition, medications, self-care skills, and emotional needs.Coordinated Care Clinic lead RN for chronic disease education for patients with CHF/COPD.
