Social Worker within the Hospital Social Work Team. The Hospital Social Work team functions within the wider Integrated Discharge Service, which consists of health professionals and social care professionals. My role within the team was to work to facilitate safe, timely, and effective discharge of patients from the acute hospital. I was expected to ensure that patients on my caseload were discharged from hospital with appropriate and sufficient support, once they are medically well enough to do so and no longer required care from an acute hospital setting. This was done within the confines of the legislative framework of the Care Act (2014) in order to prevent delayed transfers of care. I worked primarily with patients who had cognitive impairments, assessing their Mental Capacity to make decisions regarding their care, health, and support needs, and working in their best interests if they lacked mental capacity. Working within the legislation set out in the Mental Capacity Act (2005) when making decisions in the best interests of a patient, this involved facilitating best interest meetings, in which Health and Social Care professionals would be in attendance as well as the patient (if appropriate). All practicable measures to hear the patient's views would be taken to help assist in making this decision. A significant aspect of this role involved carrying out person-centered practice, ensuring each patient's feelings and wishes were heard (supporting them to return home opposed to being admitted to a care home, for instance) even if this meant challenging health professionals within the hospital, the patient-centered care remained paramount when delivering support and services.Reason for leaving: contract ended.

My role at Developing Health and Independence (DHI) was within the Engagement Team. DHI is an organisation that aims to support people who are struggling with addiction, drug misuse, mental illness, or homelessness by offering one-to-one support with a key worker, support groups, and recreational activities to promote wellbeing such as arts and crafts and yoga.The organisation also offers people practical support, such as testing for blood-borne diseases, support with criminal justice procedures, and referrals to the Riverside Clinic where methadone or Subutex prescriptions can be arranged, or the Needle Xchange program can be accessed.The service user groups I supported were diverse and accessed support for a range of different addiction and mental health issues. I worked with service users who suffered from complex mental health issues such as multiple-personality disorders, and service users who struggled with self-harm addiction.I co-facilitated and independently facilitated a weekly support group which focused on maintaining emotional wellbeing. Some of the significant topics I presented during the group sessions were around emotion regulation, managing self-injurious behaviours and suicidal ideation.My work with service users was centered around The Stages of Change; a cyclical model that aims to establish where an individual is located in their journey towards change and rehabilitation, as well as the implementation of the task-centered approach when service users presented having relapsed or in acute distress.Reason for leaving: contract ended.
