Initial Transitional Services Program Lead
CurrentManage a caseload of the Initial Transitional Service across my local region and outlying areas. Ensuring consistency with the Corrective Service NSW practice guidelines and best practice, quality service provision. • Work in close partnership with Corrective Services NSW, building a cohesive, responsive and client focused multidisciplinary team. Whilst developing, implementing and maintaining local community networks, programs and plans in line with the goals of the Initial Transitional Service. • Support individuals with complex needs (such as mental health, acquired brain injury, alcohol and other drugs) upon initial exit from custody. • Work with individuals for a period of up to 12 weeks during the critical transitional period to support their reintegration into local community through linking with identified services, building capacity in the individuals and developing a network of supports.
