As an acute care physical therapist, I practice within defined standards of care and direct the practice of support staff in order to achieve patients’ functional and discharge goals. I am responsible for the assessment, treatment, planning, implementation, education, communication, and demonstration of leadership to address the needs of patients and their families as well as to foster a positive working relationships among professionals and ancillary staff.

During my time at this ten-week clinical experience, I worked with patients in an outpatient orthopedic setting as we aimed to provide the best possible patient care by integrating both existing and emerging clinical evidence with patient preferences and clinical experience to provide high quality care to each and every patient. I also worked to hone my observational skills in regard to gauging patients' reactions to certain exercises to determine pain and challenge levels to ensure proper progression. Lastly, I was responsible for all aspects of patient evaluation and developing customized treatment plans for each individual patient.

I am responsible for the verification and collection of patient demographic and insurance information by direct data entry to the electronic medical record during the scheduling/pre-admit or admission process prior to discharge. I also conduct an interview with the patient or authorized representative to secure information specific to requested services; accurately documenting the discussion and other registration/scheduling activities in the encounter.

Throughout this ten-week clinical experience, I worked with patients in the ICU and PCU as we strived to help them gain functional mobility through interventions such as bed mobility, transfers, and ambulation. I was also responsible for recognizing important contraindications and patient acuity when assessing patient care and treatment plans, such as vitals, lab values, pathophysiology, etc. that help to determine appropriate physical therapy for each individual patient. Lastly, I was tasked with understanding and implementing discharge planning recommendations that encompassed the need for long-term rehabilitation, durable medical equipment, or home care services.

Within my time in this ten-week clinical experience, I cared for a variety of patients in an outpatient orthopedic setting as my team and I worked to provide the highest levels of patient care by utilizing new and existing clinical evidence, patient beliefs and culture, and providers’ clinical experiences to provide the best care to each individual patient. During this time, I also worked on my observational skills to ascertain patients' reactions to our interventions to determine comfort and perceived difficulty to ensure proper progression. Lastly, I was in charge of all aspects of patient evaluation, as well as developing customized treatment plans for each unique patient.
