Assist in department of education for clinicians in Home Health, Hospice and AIM (Advanced Illness Management). Computer based live orientation education for all new clinicians. Primary instructor for all home health eligible clinicians for OASIS training. Collaborate with leadership in all service lines for identified education needs. Special projects as assigned. Nursing preceptor’s support and point of contact with clinical education department. Screen potential nursing preceptors prior to formal interview with branch leadership.

Lead and supervise a multidisciplinary team providing care to patients receiving hospice services. Collaborate with staff/clinicians for providing timely and best care in the community. Responsible for monthly staff and discipline meetings. Annual reviews of clinicians. Supervisor on-call responsibilities to support clinicians and patients for multiple sites. Point of contact for community medical supplies, durable medical equipment and pharmacy services. Interview and hire new staff in collaboration with hospice administrator.

Supervise nurses who provide telephonic case management to patients within the AIM program, ongoing education of staff, participate in education development, teach in small and large groups, help implement processes for patient centered care, clinical expert for teaching content, interview and hire new staff. Special projects including presentation of a poster at the California Compassionate Caregivers symposium (2015), collaborating with Sutter affiliates for Novato Care Management (NCM) sepsis and pneumonia for reducing hospitalizations and bundled payments

Sutter North

As a registerd nurse I worked as a case manager providing direct patient care in home health. It was during my 12 years at Sutter North that I discovered an area of nursing that I found very rewarding. Working in palliative care, helping patients to have symptoms managed for better quality of life. In this job I provided care to patients in their homes. Teaching on disease processes, medicaitons, home safety, wound care and other medical procedures as ordered by their physician.
