Assists hospice patients by providing pain management, symptom management and end of life care in accordance with accepted standards of practice and Hospice of Michigan guidelines.• Responsibilities include assessment of the appropriateness of admittance to program, creation of a plan of care, implementing plan including management of symptoms, ordering of appropriate equipment, emotional support, and timely communication with attending physicians, other hospice staff members and professionals from social services, occupational and rehabilitation therapy, and dietary services as needed to coordinate optimal care and use of resources for the patient/family.• Performs advocacy and oversight of hospice services based on patient self-determination to assure patient and family/caregiver satisfaction.• Educates patients and families on disease processes, treatment plans, medication, and doctors’ orders.• Provides RN pronouncement at time of death including notification of funeral home, hospice team, attending MD and additional family members as needed.• Comprehensive knowledge of hospice nursing process and home health care practice and CHAP Accreditation.
