Conduct screening and phone interviews of prospecive patients.Complete assessments for diagnosis, social history on adolescent and adult patients, determine diagnosis of addiction or dependency and make appropriate recommendations. Develop appropriate and therapeutic individualized treatment plans with each assigned patient.Participate in group and one-to-one therapy.Prepare and deliver lectures to patients and families on various aspects of addiction.Act as liaison with other agencies concerned with rehabilitation. Perform all 8 functions as described in the Wisconsin Counselor Certification standards.
