Inpatient Pharmacist (Prn)
Current• Monitor and manage hospitalized and ambulatory patients under facility protocol to deliver optimal cost effective therapy management• Participate in management of acute and chronic disease states, as well as medical emergencies if required• Review laboratory results and patient charts in order to detect potential adverse drug reactions and allergies• Analyze patient response to therapy, and assist medical personnel with questions regarding patient care• Answer medical personnel's questions related to medication information, drug therapy alternatives, and pharmacokinetic dosing • Document (i-Vent) communication with health care providers related to patient therapy• Complete lab orders, drug dosage adjustments, drug administration schedules under pharmacy protocols• Enter, review, and verify physician orders in Epic computer system• Verify accuracy of medication preparation prior to dispensing • Assure strict aseptic techniques, storage, and proper record keeping is followed • Verify sterile compounding products and TPN• Report and document medication errors • Advise patients of drug use, storage, common side effects, and drug interactions• Provide instructions to pharmacy technicians, interns, and residents to maintain high level of performance and quality assurance
