RN with the Immunization Outreach Education & Partnership Program: An initiative dedicated to achieving critical objectives within the community by reducing missed vaccine opportunities, increasing vaccination rates and decreasing the rate of vaccine-preventable diseases. We are committed to fostering new and innovative partnerships with the community to promote vaccine awareness and health literacy.Through targeted outreach efforts and partnerships with TVFC and ASN programs, IOEP strives to boost vaccination rates in underserved communities while playing a vital role in improving public health outcomes.

Role centered in developing, assessing and facilitating comprehensive care management activities tailored to meet physical needs of members.Assessment and care planning: evaluation of needs and identification of barriers while addressing SDOH. Collaboration and coordination: development of care plans and close collaboration with providers to ensure person-centered approach to care delivery.Monitoring and follow-up: continuous monitoring of status and progress. Ensured care plan remained aligned with member goals and evolving needs.
