Conduct psychosocial intake assessments; provide education on various mental illnesses and associated symptoms. Create individually tailored treatment plans; provision of Case management for mentally ill and severely mentally ill population; facilitate group therapy as well as individual counseling. Risk assessment and safety planning for IPV/DV survivors; crisis intervention. Collaborate with providers (medical PCPs, legal, eligibility workers) and outside community organizations (DHS workers, probation officers, etc) for a wholistic approach in assisting and catering to individual needs. Advocate for individuals within the community. Certified Tobacco Treatment Specialist providing individual counseling and education on Nicotine Replacement Therapy to quit tobacco use.
