In nearly 2 years of operation, Trace touched over 2.2 million cases and 1.8 million contacts-- more than 30% of the New York population. I wrangled the data behind all those calls using R in order to:- Evaluate program metrics- Guide operational decisions by senior Trace leadership- Respond to press requests- Manage both urgent and long-term analyses for programmatic decision making, academic research, etc.Our team regularly presented results to a wide range of both technical and non-technical audiences.

CONVINCE is a global initiative to coordinate research and communication strategy development to create support for COVID-19 immunization and the science behind it. As a contract consultant for CONVINCE USA, I conducted research and provided analytic support by creating data visualizations, conducting literature reviews, creating easy-to-understand strategy communications grounded in science, writing and editing health communications for a variety of audiences, and contributed to planning and development of a dashboard of state and local COVID-19 vaccine data. The information and resources are distributed to business leaders, funders, academics, and civilians in order to increase COVID-19 vaccine confidence and uptake.

I worked on a team of independent contractors writing progress notes for therapists and paraprofessionals. We wrote all notes following strict Medicaid documentation requirements, in a way that makes them understandable to both the staff member calling in the session or any auditor reviewing records. Each note was written within 48 hours of the staff calling in the information.

RubiconMD democratizes medical expertise by allowing PCPs to connect to specialists for econsults. Using large datasets from CMS and the Census, I conducted market research for the growing Medicare and Medicaid econsult markets and presented business strategy recommendations to senior leadership. This work was done in service of the MPH fieldwork requirement.

Carrying a caseload of 50+ high-risk patients, this position is part of the New York State Health Home program funded by Medicaid. Care managers improve health and lower costs by: coordinating care for medically complex patients, thoroughly assessing patients for strengths and barriers to care, developing health and life goals using motivational interviewing, communicating with patient's providers, developing a comprehensive care plan for each patient, facilitating social service referrals and applications as needed, conducting home visits, and more. As a result of this program, as well as other efforts across the network, readmissions have dropped by over 10%, unnecessary ED visits have been reduced, and care is less fragmented and more cohesive for patients in the program.

In this newly-created role, I assisted the Lutheran Family Health Center network in exceeding Patient Centered Medical Home standards by: conducting pre-visit planning, identifying barriers to care for patients with diabetes, helping low-income patients obtain medications or diabetes supplies, regularly following up on patients with uncontrolled diabetes, recalling no-show patients to the clinics, scheduling necessary specialty exams and appointments, interpreting during appointments, and more. In working across departments with other employees, Lutheran achieved Level 3 PCMH status, the highest possible, across nearly all sites.

This position was funded by the NYCDOHMH. It was a pilot grant intended to increase screening for colon cancer through colonoscopy referrals. Cancer screenings increased throughout the Lutheran network, and we provided valuable feedback to the DOH on how to improve the workflow and electronic medical record documentation for implementation of the project at other clinics.

National service through Community Healthcorps with Lutheran Family Health Centers' Department of Care Management. I worked primarily with the Medication Assistance Program, helping patients without health insurance gain access to free prescription medicine. We obtained medications for approximately 50 patients a month, saving each patient an average of $2,400 a year.
