We offer a supported environment within our organisation, which is a culturally responsive service connecting Aboriginal and Torres Strait Islander people to the health and SEWB services they need. We deliver great programs in the community such as Aboriginal health checks in schools where we are connecting students and their families with the care they need. We also deliver services that supports Elders and those living with a disability. We are always on the lookout to develop further partnerships that can support ours and our partners aims to deliver culturally safe care. A key aim of our service is to support the delivery of services that better support chronic disease prevention and management by connecting patients to both internal and external programs such as the Integrated Team Care (ITC) program. Our ITC teams support Aboriginal people to access coordinated care and connect well with their health care and other providers.
