Oversee coordination of care within and between primary care offices, primarily Family Medicine but also Urgent Care, Internal Medicine, and Outpatient Behavioral Health. Over 15 outpatient practice sites primarily in urban and suburban areas, but also rural settings. Busy practices and partners. Seeking to build high functioning teams to meet the current and future needs in medicine, including new paradigms of value based contracts, PCMH, MACRA, and more. Working inter-departmentally to improve coordination of care, both ambulatory and acute, and finding unique opportunities to improve access to and quality of care in primary care and urgent care. Additional work with quality and data committees for ACO and CIN, quality for clinic system, provider compensation including incentive compensation, peer review, development of communication strategy, and other important strategic work.
