Clinical Care Coordinator
CurrentAs a Clinical Care Coordinator, my role is still associated with the Expanded Community Services (ECS) program at Sound, but primarily focused on serving as the primary mental health clinician at a local partnered skilled nursing facility (SNF). My role is centered on enhancing the psychological well-being and quality of life for our geriatric population of clients tiered with Sound's ECS/EBS & ENFPP programs. Specializing in geriatric mental health, I bring a compassionate and comprehensive approach to address the unique mental health challenges faced by older adults, promoting resilience, emotional well-being, and a sense of purpose. Some of my responsibilities include providing individual therapy sessions for geriatric-aged clients, which include addressing a range of mental health issues such as depression, anxiety, grief, and cognitive decline. My services focus primarily on clients diagnosed with schizophrenia, schizoaffective disorder, and traumatic brain injuries (TBIs) residing at the SNF. There is also a responsibility to collaborate with staff at the SNF and the Washington State Department of Social and Health Services (DSHS) to provide support and education on managing the mental health needs of these clients who reside at the SNF.
