Responsible for care of forty consumers’ mental health needs. Coordinate Consumer’s developmental disability, psychiatric, medical and environmental needs. Assist with referral and linkage to community resources while being an advocate for the consumer. Continual assessment of Consumer’s needs and development of individualized Service Plan. Respond to urgent/crisis Consumers’ needs as appropriate with crisis services and clinic staff by developing plans and interventions. Develop and maintain a relationship within family unit and friends. Participate in daily meetings to coordinate the consumer’s care including the Mental Health Provider and Nurse. Meet individually with each consumer at least twice a month at his/her home and once a month at the clinic to help achieve planned goals. Work as part of a team to meet the consumer’s needs and improve their lives.

Responsible for observing clients and maintaining documentation for review. Responsible for client activities including off- premises meetings and activities. Supervise/council young women at the ‘Ladies House.’ Responsible for recording clients taking their medicines in the (MAR) Medical Administration Record. Responsible for orientating new hire MHTs. Assist in admitting new clients. Responsible for auditing charts and notifying the clinical program director, counselors and other staff of chart deficiencies.

Seven years experience as a MHT working on the Adult Psychiatric Unit, Adolescents and the Dual Diagnosis Unit. Responsible for training new-hire MHTs. Experience assisting the Options Department assessing patients for admission. Responsible for maintaining order with patient activities including leading the End-of- the-Day Group Session. Communicated with nurses and psychiatrist regarding patient’s progress. Responsible for inputting patient vitals, activities, progress and concerns into the computer using Cerner and Meditech system.
