• Support the design, implementation, M&E, data use, CQI, reporting, and documentation of adult HIV care, treatment, and TBHIV activities and Work with CHMTs and other ICAP staff to build capacity and mentor facility providers on implementing program priorities, such as timely ART initiation for newly identified HIV clients, viral load suppression, establishment and management of viremia clinics for clients with high HIV viral load, including enhanced adherence counseling (EAC) fidelity, intensive TB screening and case finding among PLHIVs, IPT initiation, and completion, and all other interventions as per the national and donor guidance. • Work with district teams to implement innovative, data-driven, quality adult HIV care, treatment, and TBHIV services, including providing male-friendly services and support for PLHIV support groups.• Provide technical assistance (TA) to the CHMT members and support facilities in planning, implementation, M&E, and reporting of adult HIV care, treatment, and TBHIV interventions.• Collaborate with CHMT to assess and implement initiatives to address skill and resource needs in adult HIV care, treatment, and TBHIV services through supportive supervision, mentorship, on-job training, and design remediation strategies using CQI methodologies• Ensure compliance with national and international standards and best practices for program implementation.• Participate in the data collection, analysis, and use at facility and district council levels and contribute to the development of monthly, quarterly, and annual reports • Lead the documentation of program success stories and lessons learned in the relevant technical areas.• Coordinate and facilitate the monthly meeting for sharing challenges and strategies for the improvement of adult HIV care, treatment, and TBHIV activities• Support sites to understand targets, align their strategies and activities towards them and meet their set performance targets.

*Project: Improving Health and Well-being of Adolescents and Young Adults HIV+ through a research and health system strengthening approach in line with the National Accelerated Action and Investment Agenda for Adolescent health and well-being (NAIA- AHW) 2021/22 –2024/25 and the Health Sector HIV and AIDS Strategic Plan (HSHSP IV) (2017–2022)My responsibilities include:• Planning of project activities such as designing sensitization and awareness raising activities for adolescents and young adults in Shinyanga region on HIV/AIDS, STI, SRHR and GBV• Monitoring & Evaluation (M&E) – data collection at healthcare facilities’ level on access to services, enrolment and adherence to ARV therapy in Shinyanga MC and Shinyanga DC• Coordinating technical meetings with local, regional and national partners (including international and local NGOs, MoH, NIMR, NACP, and TACAIDS)• Planning and implementing the strategy to improve the quality of HTS and treatment of adolescents and young adults, to increase the number of patients and their retention in care in the target CTCs•Supportive supervision of target CTCs including PMTCT and adolescent clinics• Capacity building to HCWs in supported facilities on adolescent friendly approach in managing HIV+ adolescents and young adults accessing services• Preparation of project reports

among my duties are;• Reviewing all aspects of fieldwork, including management of team leaders and field workers, on a daily basis. • Ensuring that field teams adhere to THIS protocol and Standard Operating Procedures and maintaining adequate and accurate participant records, as per the study protocol.• Determining readiness for survey implementation in each enumeration area (EA) and complete pre-implementation steps and also ensuring availability of all necessary equipment that are working properly.• Overseeing transportation of field samples to the Satellite Laboratory and/or National Reference Lab. • Assessing ongoing performance of Field Team Leaders and Nurses/Interviewers and identify training needs while also performing EA and household spot checks.• Participating in community mobilization activities, including district launches and public gatherings and planning logistics for fieldwork data collection.• Verifying and ensuring data quality by performing completeness and consistency checkswhile reporting to supervisor weekly

achievements included;-Visited over 54 supported facilities and vaccination points, mentored on various ways of maintaining cold chain and increasing covid-19 vaccine uptake and at the end attained 86% of fully vaccination for PLHIV from 41.9% in 3 ½ months.-Created a good flow of quality data and posted Taskforce in nearly each facility hence facilitating a zero backlog of vaccination data entered in chanjocovid.among my duties are;• Working with health facilities and community leaders to develop and rollout micro-plans for accelerating COVID-19 vaccination uptake • Supporting health facilities to establish temporary fixed vaccination sites at the community level and Providing daily support to health care workers (HCWs) on delivery of COVID-19 vaccination services in both Health facilities and community settings.• Ensuring proper documentation in registers while the vaccination services are ongoing.• Leading in the development, implementation and monitoring district and health facility level plans for COVID-19 vaccination and leading in the COVID-19 vaccination data review meeting with CHMTs and support the utilization of available data by healthcare workers and support staff to scale up vaccination efforts.• Managing timely implementation of the COVID -19 vaccination activities according to the objectives and deliverables, conducting relevant site visits, supporting district team• Performing any other tasks as assigned by supervisor

Achievements attained• Oversaw successfully 3 supported care and treatment sites for coaching and mentorship on clinical care of PLHIV according to National HIV/AIDS national guidelines.• Successfully liaised with laboratory and supply chain management team to support supply chain management services and ensured HIV commodities at all levels within the districts• mentored and guided retention modalities and achieved 96% from 74% in Mwananchi trust hospital, covid vaccination from 6% to 76%among my duties are;• Working with ECs assigned to the facility to arrange clients’ records in a smart and easily accessible manner.• Preparing lists for clients with appointments using the appointment register, a day before and ensure their files have been identified and placed at an accessible location and Making phone calls to clients for appointment reminder (at time intervals including 1 week, 3 days, and 1 day prior to appointment date) and for unreachable clients share information with EC for physical reminder • Arranging ART refill differentiated service delivery modality (DSDM) in collaboration with CTC HCWs and ECs based on clients need and as per national guidelines and Supporting linear scale up of 6MMD to eligible all eligible clients in the HF and Returning files to their respective location after the data clerk has entered the information into the database • Sorting and marking clients for HVL eligible clients prior to clinic day and remindingclinician/nurse to request for a test when the client comes for their appointment and collaborating with HCWs, update appointment register during clinic visits • Sorting files for Missaps and clients with interruption to treatment, sharing list for training with ECs and updating status in tracking register together with Sorting and removing files for clients that have been transferred out or with reported death status and organizing them for easy access

Among my duties included• Managing opportunistic infections, complications of ART and staging HIV/AIDS clients.• Providing consultation of HIV-positive clients in the wards as requested and maintaining accurate medical records and other data collection as required by Monitoring and evaluation within clinic• Preparing and participating in medical ward rounds and overseeing that prescribed instructions are carried out.• Participating in clinical sessions, patient presentation, journal clubs and attending emergency medical duties and responsibilities.• Conducting teaching/coaching of the departmental staff and students on new medical updates.• Attending inpatients and outpatients• Attending CTC and mother and child health clinic and COVID-19 patients• Educating on prevention of HIV• Performing major and minor surgical procedures• Instructing and teaching medical and nursing students
