Outpatient Case Manager

Serving the MI population in the community and in residential settings. Providing case management services including person-centered planning, writing treatment plans, assisting with housing, transportation, obtaining public assistance, employment, education and training, and obtaining authorizations from county mental health agency (MCCMH). Completed Incident Reports, petitioned for hospitalizations (consumers with AOT/ATOs), made referrals for residential placements and CLS services, and visited with consumers in the community(private homes, group homes, room-and-boards, 3/4 houses, hospitals, and various community businesses). Worked with internal and county EMRs, completing authorizations, ISOPs, periodic reviews, progress notes, chart notes, and completed various forms (including "Consents to Share Behavioral Health Information", "Acknowledgement and Consent to Treatment", "Advance/Adequate Notice" Due Process letters, etc. Completed LOCUS assessments and employed Columbia Suicide screenings. Coordinated treatment with multi-disciplinary team members, including psychiatrists, nurses, therapists, and managers. Trained in Recipient Rights, Self-Determination, Medicaid Fraud, and patient privacy (HIPAA). Carried average caseload of approximately 50-70 consumers.
