Served as the Care Coordination and PHM Lead for a New York State Delivery System Reform Incentive Payment Program (DSRIP) designed to reduce Medicaid costs by improving the health of populations and coordinating care through an integrated provider network. Primarily focused on the technology used to integrate primary care, behavioral health, and ED providers and the processes by which patients were engaged and navigated through the care continuum.
