Clinical Case Manager, Permanency Program (Therapeutic Foster Care)
Provide case management and weekly home visits to therapeutic foster homes. Collaborate with youth and foster families, as well as DCF workers, medical providers, mental health clinicians, teachers and other school staff, education surrogates, and any other important people and providers in their lives. Through these connections, provide support for areas including but not limited to permanency planning, treatment planning, success at school, other appointments, medication regimens, and overall family functioning. Build and maintain therapeutic relationships with children and their families to effectively support these areas. Provide psychoeducation to foster parents to help them better understand their children’s diagnoses and behavior that are a result of trauma. Crisis management is constant between finding respite placements, intervening in behavioral crises, and providing resources to prevent disruptions. Also responsible for regular updates of monthly progress notes, quarterly service plans, biopsychosocial assessments, individualized crisis management plans, serious event reports, PIE data, and discharge plans.
