Clinical Care Transition Specialists Ensure patients are properly placed on home IV infusion services, including patient assessments, extensive patient education, and medication administration/ hook ups pre discharge. Support inpatient case management within partner facilities by gathering preadmission information, collaborating with internal (intake) and.

• Follow members for 30 days post admission ensuring optimal transition from acute care to home, SNF, or other disposition.
• Collaborate with multi disciplinary team members to ensure safe and appropriate discharge planning.
• Conducts face-to-face visits of all members while in the hospital and home visits of high-risk members post-discharge.
• Educates and supports member focusing on seven primary areas (ToC Pillars): medication management, use of personal health record, follow up care, signs and symptoms of worsening condition, nutrition, functional needs.
• Ensures member transitions to a setting with adequate caregiving and functional support, as well as medical and medication oversight as required.
