Serving as key point person for inpatient care coordination, transitions of care and linkages post discharge. Perform extensive root cause analysis of potential barriers to stabilization and then integrate with the cross continuum and team to link established care management relationships. Engage the care team with information to drive the care plan across the continuum.

Responsible for providing Care Coordination services within the primary care setting, for patients who are considered at risk and who experience barriers to healthcare. Care coordination services include: providing episodic and longitudinal care, monitoring acute facility stays and discharges that facilitate smooth transition of care across the care settings, facilitating diabetes education groups, individual diabetes education, chronic disease management, lifestyle coaching with emphasis on preventing diabetes, empowering patience with self management skills to improve health.

Responsible for ensuring high quality customer service and disability case management, by providing medical expertise and serving as critical communication link between all parties involved in medical disability cases.

Conduct 1:1 coaching and counseling with clinician on assignment to inform on policies, mediate problems, reengage to assignment or reassign if applicable. Respond urgently to potential clinician and client issues that jeopardize assignment completion by coordinating with internal sales and operations leaders to make timely and informed decisions that address customer need while aligned with business objective. Troubleshoot clinician assignment issues by investigating all relevant information/data and evaluating situation using conflict management & problem-solving tactics in order to determine best win-win outcome to improve performance and make termination decisions of necessary. Support local staffing branches from clinical perspective to partner on clinical review process and maintain excellent quality of clinician on assignment with facility.

Identify new patients electronically and enroll in education program. Provided education telephonically to dialysis patients during the first four months of treatment. Collaborate with Clinical managers as needed to alert to possible issues and/or to enlist their help or support.

Work Collaboratively with the physician and anesthesia health care providers to determine patient needs during Endoscopic procedures and to assume responsibility for patient assessment and care, document and evaluate effectiveness of nursing actions and ensure safe environment of care. Provide technical assistance during diagnostic and therapeutic Endoscopic procedures ensuring safe working practice. Post procedure recovery and discharge planning.
