Patient Advocacy healthcare for special populations who have a strong desire to stay in their home as they age with special needs, and to learn how to increase the quality of their lives. Populations include those who have rehabilitation needs following traumatic life changes, chronic conditions, mental health, enhancing life during old age, have rehabilitative needs as they recover from surgeries, chronic conditions due to non-communicative disease, stroke, balancing issues/fall prevention with environmental modification, those living with arthritis, needs of the aging, those in need of a caring and compassionate companion, those in need of physical/social/cognitive rehabilitation, Transportation needs, with client chosen goals that optimize health and higher functioning on many levels, for all ages. I have 15+ years of empowering and patients through in home healthcare needs, with the tools, resources, and medical support they need. This includes customized interventions to improve the person’s ability to perform daily activities and reach the goals, to treat the physical, mental, developmental, and emotional ailments that impact a patient's ability to perform day-to-day tasks to help these patients develop, recover, improve, transportation needs, as well as maintain the skills needed for daily living and working. The goal is to help clients to address many dimensions of disability, including physical, psychological, cognitive, and social that prevent from engaging in meaningful daily occupations. Individualized evaluations can be given based on interests, needs. This is done by personal collaborating to determine the person’s goals for a higher quality of life and integration. Ultimately, the goal is to provide clients with the tools they need to best function within their daily world.I am a compassionate Patient Advocate that uses an Occupational Therapy structure of meeting health care and quality of health needs, I interact with clients across all communication channels (phone, chat, email, messaging, and online portal). In my skillful therapeutic approach, I can work closely with medical teams to ensure clients receive the care they require, and the information they need to make the best healthcare decisions for themselves and their families.Through collaboration and therapeutic assessment, I can help with the barriers that affect a person's emotional, social, and physical needs using everyday activities, exercises, and other therapies to achieve a higher and esteemed quality of life.
