I have been working with a diverse range of young people, focusing on successfully transitioning them from the care of the Chief Executive into stable adult accomodation. While supporting them to obtain everything they need to live independently, such as education, employment, how to access services and financial supports. All while following the legislation and ethics that are expected of me. In my role I also facilitate family contacts between children and their families. I also undertake administrative duties such as case planning, phone calls, emails and scheduling. Note taking on C3MS is also a skill I have obtained. Given the nature of my role, building and maintaining relationships is vital, I work closely with schools, birth families, health, carers, NGO’s, NDIS and of course my clients. With these relationships difficult conversations occur and I manage these well. I am culturally aware through out my practice and I am always learning more each day about the diverse range of cultures with in my community.
