- Facilitating admissions and discharge planning-Counseling residents and family members- Completing all required documentation in the medical record- Including MDS, care planning, discharge planning and being an integral member of the interdisciplinary team.

•Process all intake referrals (walk-ins, fax, phone, email on a daily basis)•Maintain a caseload of an average of 50-60 clients and conducted adequate outreach•Obtain all pertinent information to process the referral including history of psychiatric/mental health treatment, assess for any developmental delays, explore academic/work-related difficulties and review criminal/drug history.•Scheduling client appointments in electronic health record (IMA), including ADM and PE.•Verify insurance eligibility to ensure that a potential client has proper medical coverage•Collaborate with referring agencies and provide an update on the status of a referral•Obtain and review Foster care referral packages and hospital packets prior to scheduling an intake appointment•Discussing and reviewing hospital referrals with administration prior to scheduling•Aid therapists as needed with referrals for developmental testing or other social service programs.

- Demonstrated understanding of RN Case Management Services and ability to practice critical thinking when evaluating referrals for post hospital care.- Coordinate services between home health care, hospitals, DME agencies and the medical community so that continuity of care for patients is maintained.- Obtain/receive referrals and orders from physicians, hospitals and other referral sources. Established relationships with community referral sources and supports staff relationships of a like nature.- Act as facilitator regarding communication between appropriate personnel of the agency, hospitals and other referral sources.

- Watch for and meet signs of emotional or developmental needs in children and bring to the attention of parents- Organize activities and implement a curriculum that allow children to learn about the world and explore interests- Develop schedules and routines to ensure that children have enough physical activity, rest, and playtime- Assist in implementing daily program and preparing the learning environment materials and supplies- Assess children and maintain individual child portfolios as well as other classroom duties
