200 + bed acute-care facility (includes ER, level II trauma center, NICU)• Responsible for monitoring, evaluating and making recommendations to assure the appropriate and effective use of pharmaceuticals for patient care.• Verify new/revised inpatient (Med/Surg, Rehab, Oncology, Tele, ICU, IMC), OR, ER orders, and IV compound, enter/dispense urgent OR orders, crash cart and tray checks, NICU fluids and TPN order checks, and clearing out clinical interventions.

Facilitate support for 99 bed hospital.• Accurately review, verify, prepare inpatient medication orders.• Perform as medication reconciliation pharmacist.• Play key role during transition from Rehabilitation hospital to Acute hospital: Review formulary, set up ICU, MDSG, Preop, PACU, OR, and GI pyxis, and set up C2 safe.• Lead Antimicrobial Stewardship Program.• Main preceptor.• Updated all IV compounding logs to match new BUD requirements.• Main pharmacist to review narcotic use in C2 safe, and reconcile all IV compounding logs for control substance.• IT core member with Director of Pharmacy and IT Pharmacist while transitioning to a new pharmacy program.• Assist the Director of Pharmacy in implementing new regulation and computer system update.• Counsel patients concerning inpatient medication or during discharge.• Educate, train, and supervise pharmacists, pharmacy students, and volunteers in their job tasks as needed.

Facilitated support for 224-bed hospital.• Accurately verified inpatient medication orders including chemotherapy and Total Parenteral Nutrition (TPN).• Input adverse drug reactions, interventions, and medication errors into Siemens pharmacy system, achieving daily goal of entering 100% of data on the same day.• Attentively monitored 60-70 inpatients’ laboratory and drug levels with corresponding dose/frequency adjustments.• Counseled patients concerning medication prior to discharge as requested by medical team.• Verified preparation of medication in central pharmacy prior to delivery.
