Iris Consultant
IRIS Consultant assisting individuals and families with their long-term care needs. Also worked alongside participants in the State program known as IRIS to create and review their plans and how to allocate their yearly budget. Aided participants in finding affordable housing, jobs, schools, gyms, and hiring of employees that can provide supportive and personal care services. Created an emergency backup plan specifically for the participant to capture their emergency contacts such as doctors, nurses, hospitals, and other relevant individuals that can aide them upon an emergency. Conducted Quarterly Home Visits with participants to ensure that they are indeed getting the most qualified level of care from their employees that are providing supportive and personal care services. Also monitored participants budgets on a monthly basis to ensure properly usage of State funding and requested a budget amendment or one time expense to cover anything that would exceed their yearly budget.
