The Monroe County Systems Integration Project (SIP) is an initiative to develop and implement improved, person- centered service delivery for individuals who are vulnerable or living in poverty. A key component of this work is the redesign of the formal system’s current service delivery model, including, designing and implementing new workflows, strategies and tools that allow service providers across health, human services and education to work together in a more coordinated fashion.The Systems Integration Project Office is directly responsible for successful implementation of the Monroe County SIP, including developing and managing a robust Provider Network to: o Define and maintain community-will for project implementation o Implement, monitor, and modify solutions to achieve community good o Foster trust, shared accountability and transparency across new and existing workgroups, committees, institutions and vendors o Engage 10,000 community members in the system redesign processI have the honor, privilege and opportunity to be the lead in being responsible for growing and maintaining an intentional, diverse and comprehensive Provider Network for the project!
