Completes weekly functional assessments and engages the PAC inter-disciplinary care team providing them with the nH Outcome tool to align expectations for discharge planning. The position engages patients and families to share information and facilitate informed decisions. By serving as the link between patients and the appropriate health care personnel, the SICC is responsible for ensuring efficient, smooth, and prompt transitions of care.

Responsible for the overall direction of home health clinical services, quality, adequacy of services provided and supervision of staff, establishes, implements and evaluates goals and objectives for home health services that meet and promote the standards of quality. Assures that care is coordinated between services and that all of the client’s needs are identified through the assessment process and addressed through the implementation of the Plan of Care and supervisory process. Makes the initial evaluation visit and regularly reevaluates the client’s nursing needs. Initiates the plan of care and makes necessary revisions. Provides those services requiring substantial and specialized nursing skill. Informs the physician and other personnel of changes in the patient’s condition and needs. Counsels the client and family in meeting nursing and related needs. Assists in the development of organization goals. Develops, recommends, and administers agency policies and procedures.Prepares and maintains records in accordance with Joint Commission standards. Assesses competency of clinical staff and ensures staff is assigned in a manner that promotes quality, continuity and safety of a client’s care. Supervises, educates and coaches other nursing personnel. Plans and implements in-service and training programs. Conducts performance evaluation of medical care-giving staff. Oversees the maintenance of client clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities. Assures proper maintenance of clinical records in federal laws. Identifies and participates in performance improvement activities. Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel. Promotes home health referrals in the health care community.

Manage the coordination, supervision, and implementation of professional and supportive services to clients in a safe, cost-effective manner. Responsible for the coordination of care during the episode of health care treatments. Provide education and training related to clinical practice issues and regulation and reimbursement changes with new employee during precepting phase of new hire orientation.

Supervisor for 27 PHP/FPG Case Managers and 3 Affiliate Case Managers. Coordinate post-hospitalization needs for members identified as being at risk of re-hospitalization or as frequent users of high cost services for approximately 7500-8000 patients annually. Oversee timely physician and home health services, medications, medical equipment, oxygen, therapies and other support as required. Responsible for census management for high bed days, vent and complex care, telephonic transition of care and act as a liaison between SNF's, LTAC's, Home Health services and Medicare Advantage plans including: Wellcare, Freedom/Optimum and Care Plus. Provide training for new Case Managers. Manage Utilization cost and Train New hires

Medication management for Pulmonary Atrial Hypertension medications. Including Tracleer, Letaris, Remodulin, Revatio and Adcerca. Currently following approximately 3000 patients. Continuous monitoring through monthly assessments and progress.

Developed and administered policies and procedures for utilization control of inpatient and outside referral services. Managed the review of insurance plan criteria to facilitate the appropriate dosage and dispensing of pharmaceuticals. Assisted in the development and implementation of procedures to track and trend specialty and ancillary service utilization within the facility; Assisted in developing and piloting paperless processing for department with potential savings of $25K per year.
