CurrentAs the Population Health Data Analyst, I work closely with executive leadership and business operations to ensure information needed to improve health outcomes and lower costs is accurate, timely, and readily available to key stakeholders across the organization. Primary responsibilities include: •Work with active and potential stakeholders to understand and exchange clinical datasets to be used in operational analysis and reporting, with particular focus on the Value-Based quality initiatives and the Accountable Care Organization (ACO).•Using both clinical and claims data, develop dashboards/reports using Tableau to assist with analysis around quality measure performance and cost and utilization. This includes the development of Physician/Department/Organization Scorecards to track and manage quality measure performance.•Develop Cost and Utilization reports to provide insight into specific populations cost and utilization; including Per Member Per Month (PMPM) – Overall, Medical, Rx, Emergency Room/Inpatient/Urgent Care Utilization, and Spend/Utilization associated with Chronic Disease Management.•Participate in strategic and tactical planning for operational application development, reporting and enhancements with IT management and informatics team. •Structure data and work with stakeholders to develop and evaluate specific application functionality and enhancements of the operational datasets.
