In this role I functioned as an independent team member throughout the entire transplantation process, from evaluation of potential live organ donors and recipients to surgery and patient follow-up. I communicated with patients and donors across 20 different states as part of this world-renowned transplant center. I consulted with staff and providers in the creation of guidelines and workflows to structure the cross-functional care teams. Additionally, I served as Preceptor for 10 nursing students each year and built some very intensive clinical orientation programs. Designed a multi-disciplined approach to patient care and served as gatekeeper and strong patient advocate. While I have moved to Boston, I still work remotely for the hospital.✰ Worked directly with Jon Odorico, MD in the design of pancreas transplant guidelines to create a framework for use by new doctors, coordinators, and advanced practice providers.✰ Collaborated QAPI Discharge Improvement Workflow & Continuation of Care-Laboratory Testing, enabling the creation of best practice for lab test ordering and results communication across the inpatient to outpatient spectrum.✰ Collaborated QAPI Streamlining Outpatient Scheduling Process for Transplant Specific Patient Infusions-IVIG, Rituximab. This program increased the efficiency of arranging infusions and decreased the number of rejected insurance approval requests. ✰ Declared a program EPIC Superuser and conducted annual evaluations with EPIC representatives to disseminate information and improve internal functionality and capabilities of the EPIC EHR.
