Katy Hospice, LLC is a small-town hospice, locally owned and operated. Established by our passion for providing the highest quality of care to those facing an end-of-life illness. Our team is highly trained and experienced in making the final stage of life peaceful while patients remain surrounded by loved ones.

-Provided care to patients with acute psychosis, organic affective disorders, major depression and adjustment disorders of aging-Created individual treatment plan for each patient-Prepared and participated in group and one on one therapy sessions-Coordinated Care Conferences, which included Physician, Director, patient, family and at times APS and Attorney

-Functioned as Executive Administrator's designee in absence of Administrator-Provided leadership for 30 direct reports including RN's and CNA's-Accountable for the delivery of high quality nursing care for a 52 bed unit-Exercised independent judgement with 24-hour accountability for clinical and operational issues-Established clear clinical practice expectations for resident care

-Provided patient care; evaluated patients condition and developed a plan of care that addressed all medical and social needs-Provided pain management and emotional/spiritual support tailored to fit patients needs and wishes-Perform regular audits of patient records-Performed monthly and quarterly reviews

-Inspected Assisted Living homes conditions and operation to ensure compliance with states codes

-Promoted and marketed Amedisys Hospice to the Alaska Healthcare Community-Increased census and provided excellent marketing support-Worked closely with interdisciplinary team-Coordinated the care for patients and served as the liaison between families and physicians

-Provided leadership for 31 direct reports including Rn's, LPN's and CNA's-Accountable for the delivery of high quality nursing care for a 47 bed unit-Exercised independent judgement with 24-hour accountability for clinical and operational issues-Developed, implemented, evaluated and modified programs and services needed to meet resident care and staff development needs

-Prepared patient for visit with provider, assisted in multiple procedures-Coordinated referrals in collaboration with patient, family and interdisciplinary health care team-Educated patients/families on health care needs, conditions and options

-Provided quality care to patients and their families based on individual needs-Assessed and analyzed patient's current and past medical information with superior understanding of cognitive, physical emotional/ psychosocial, spiritual and chronological maturation process-Educated patients and families about the disease process, interventions, procedures and medications

-Assessed the nature of the illness/injury, prioritized treatment and transport-Made critical decisions while remaining calm and professional-Preformed cardiac defibrillation, intravenous drug therapy and advance airways placement-Communicated with receiving hospital and physicians via voice and telemetry communication
