I am the Project Coordinator for the Health Workforce Retraining Initiative (HWRI), I am responsible for the development and implementation of the New York State (NYS) funded grant. The purpose of the grant is to offer an enhanced training to home health aides (HHA) to assist them in providing a higher quality of care. After reading through the NYS contract and extracting deliverables and measurable outcomes I created a project plan to ensure the tasks associated with every deliverable/outcome is complete. Development:Affirming curriculum topics as stated within the NYS contract and contracting with curriculum developers to create the one of a kind training and train-the-trainer materials. A work group consisting of leadership from invested areas was assembled to ensure curriculum met the needs of the organization's members and HHAs while maintaining the mission and values of the organization. I decided the criteria to be used for the HHA selection should focus on those HHAs who service high risk members. Using this method we are able to evaluate member's health before and after their HHA receives training and therefore are able to measure the effectiveness of the curriculum and obtain training metrics. Implementation:I facilitate the training sessions by securing the location, time frame and trainers, and the HHAs to be trained. I evaluate HHAs's pre and post test scores to identify any gaps in training and filling those gaps. Using a budget tracker I created I verify the allocation of moneys and ensure proper payment of goods and services. Periodically, I analyze the rehospitalization member data to note areas or periods of improving health.
