Leading efforts in public health for Colorado Medicaid, my focus is on Medicaid expansion and reform in the Health Programs Office. I work to enhance care coordination, integrate social services and public health into the Accountable Care Collaborative, and address social determinants of health to improve health outcomes and population health for Medicaid clients in Colorado.Focus areas include, diabetes, tobacco cessation, lay health workers, nutrition and obesity prevention, immunizations, dental health, hypertension, CVD, and developing relationships between Medicaid and Colorado's Local Public Health Agencies (LPHAs).

Coordinated work with statewide partners, including employer groups, providers, payers, large healthcare delivery organizations and health plans interested in chronic disease prevention and management through evidence-based programs such as the Diabetes Prevention Program, Diabetes Self Management Education and the Chronic Disease Self Management Program to expand the scope of disease prevention and management across Colorado.

Greater Philadelphia Area

Analyzed data from the Youth Risk Behavior Survey (YRBS) from the Philadelphia School District pertaining to healthy eating and active living (HEAL) strategies. Formed and facilitated focus groups of target students from Philadelphia public schools and Philadelphia charter schools to gather access, availability, perceived barriers and benefits to HEAL activities. Created an awareness (poster) campaign that was disseminated to all Philadelphia high schools for strategies on how to live a healthier life.

Greater Philadelphia Area

Partnered with Medicaid of Pennsylvania to promote home health in lieu of nursing home care for seniors in Philadelphia. Helped empower clients to become virtual employers, hiring family or friends as their home health assistants. Acted as the fiscal liaison, case and care manager. Conducted home health visits, mentoring, and counseling of clients.
