I am the Director of Business Development and Care Transitions for All About You Home Care Collaborative Services (AAY). I have directed the implementation of Transitions of Care, A Safe Trip Home with assisted living facilities, skilled nursing facilities, rehabs, hospitals and homes. The focus of Transitions of Care is a controlled delivery model to efficiently provide optimal health care at home, improve patient outcomes and decrease recidivism. I facilitate communication between patients, families, hospital case management, discharge planners, administration and clinical staff. My expertise in marketing, customer service and community relations with multiple collaborative groups and ACO physicians has lead to significant growth and patient satisfaction for AAY. I provide consistent education, training and monitoring for licensed clinical staff which is an integral part of the success of our program. AAY is a preferred provider for home care at Griffin Hospital in Derby and Trinity Health System (St Mary's Hospital) in Waterbury. We transition patients from Yale New Haven Hospital, HSR, Bridgeport Hospital, St Vincent's, St Mary's Hospital, Waterbury Hospital, Gaylord Hospital, Charlotte Hungerford, Midddlsex Hospital and multiple skilled nursing facilities and short term rehabs. AAY collaborates with APRN House calls for acute care when are patients aren't able to get to their physician. We are leaders in decreasing recidivism. In 2013 AAY was once again named an Elite Home Care by The National Research Corporation. We have a professional, compassionate team of RN's, LPN's, Physical Therapists, Occupational Therapists, Speech Therapists, APRN's, Home Health Aides and more.
