Peer Specialist Coordinator
Current- Provide essential youth support and mentoring, acting as professional staff that liaison between teen and parent, school and community
- Support youth and family through phone contact, email and direct service. Accompany teens to team meetings, school meetings, and other appointments as needed
- Participate in county and family meetings in regards to reviews, service planning and educational needs (IEP’s, 504’s, etc.)
- Work with systems to advocate in helping families navigate systems that offer support to teens with special needs
- Have an understanding of and use WRAP, Person-centered planning and TIP when working with youth.
- Maintain client case notes, files and progress reports in accordance with agency policy, program regulation and technology
